Chronic total or near-total coronary occlusions (CTOs) represent a significant challenge for all angioplasty cases and also are the target of many interventional devices from balloons to stents to wire-based crossing systems [1,2]. Although new interventional devices continue to be designed, evaluated in preclinical models and eventually entered into clinical trials, the development of a reproducible preclinical model of a partial or near-total occlusion characterized by a calcified matrix and inflammatory infiltrate exhibiting similar physical properties to those observed in the human population has been proven difficult [3]. Others groups have evaluated complete occlusions using bone chips in gelatin or a collagen matrix, copper stents or wires [4,5].
