The analysis of sequential events is a major topic in the analysis of accidents and systems’ operational safety. Systems analysts must identify the sequences of success and failures (systemic, human, procedural, organizational, etc.) due to initiating hazardous events at the earlier phases of system design. They seek to eliminate/reduce the occurrence of the feared sequences to interrupt the order of the occurrence of the events in the sequences, and/or to mitigate their consequences.
