Abstract
The objective of this paper is to discuss the risk associated with fasteners used in the offshore industry. The intention is to suggest a way forward in establishing a robust system for management of fastener integrity on offshore installation, both topside and subsea, and thereby improve the safety level.
However, the large variation in design and use of bolted connections have to be analysed to identify the areas with the largest risk. Based on analysis of incidents reported to the Petroleum Safety Authority, Norway and analysis of industry standards and practices possible industry initiatives are identified.
